GENERAL RESPONSIBLITES: Provides case management services to 30-35 children and young adults with disabilities; coordinates the intake and eligibility process; facilitates the development of the Individualized Service Plan; assists children and their families in accessing services; coordinates and monitors provision of services; provides crisis intervention, mediates conflicts related to services; maintains case records; advocates on behalf of the children and their families.
HOURS: Business hours and other hours as needed

REQUIREMENTS:
General: Eligible to work in the Untied States
Education: Bachelor’s degree in a human services field required (at least 30 semester hours in the behavioral sciences, education, health care, human service administration or the social sciences is required with in the bachelors degree); or have an Iowa license to practice as a registered nurse.
Experience: One year experience serving children or young adults with disabilities or RN with Three years of experience serving this population.
